Notice to Medicare Patients
As a TEAM participant, The Summit is required to inform you about this program. Your care and coverage will not change. Participation is automatic for eligible Medicare beneficiaries undergoing certain orthopedic or spinal procedures.

What is the TEAM Program?

The Transforming Episode Accountability Model (TEAM) is a Centers for Medicare & Medicaid Services (CMS) initiative that encourages hospitals and health systems to better coordinate care for Medicare patients undergoing:

  • Lower extremity joint replacement (hip or knee) 
  • Certain spinal fusion procedures 

Under TEAM, The Summit is accountable for the quality and total cost of care from the time of your surgery through 90 days of recovery, including rehabilitation, follow-up visits, and related services.

This model is designed to support recovery, reduce complications, and improve overall outcomes.

What this means for you

If you are a Medicare beneficiary having a qualifying procedure at The Summit, you can expect:

You will receive written notice

Before your procedure, you will receive information explaining your participation in TEAM and your rights as a patient. You may continue to receive care from any Medicare provider.

You will receive a text message

You will receive an SMS text message from Revel Ai that allows The Summit care team to communicate with. It is important that you approve or allow these messages so your care team can stay connected with you.

A care navigator will be assigned to you

A care navigator will contact you before surgery, help you prepare, and follow up throughout your 90-day recovery period.

Your Medicare benefits remain unchanged

TEAM does not change your coverage, cost-sharing, or your ability to choose your providers. You will not be billed differently as a result of this program.

Additional care coordination support may be available

In some cases, CMS allows hospitals to provide additional support related to care coordination. Your care team will explain if this applies to you.

Support for your recovery

If you have concerns after surgery, such as a low-grade temperature, incision site changes, or increasedpain, please call The Summit nursing team before going to the emergency room.
